Built by DEKMS on each of the Hardcore Worlds for notable characters.
The general rationale is that a statue has to be earned, though the criteria for earning one are rather loose. This could be through accomplishments (or epic failure), deeds, behaviour, construction, and so forth. Some deeds such as 'the first to Slay the Dragon' obviously won't be repeated, some such as 'achieved massive bounty' are unlikely to warrant another statue for that reason, even if it tops the one already built. There is of course, nothing stopping others from building statues of themselves and others.
In terms of construction, the statues are meant to be 'carved' into a suitable mountainside - hence the grey-scale for the skin using wool, stone, and clay. The program MCSkin3D makes this very easy with tools to pull the skins from Minecraft.net, and edit them. I (DEKMS) usually use Photoshop as well to break apart the skin template and convert it to grey-scale. For added details I generally use obsidian for the eyes if suitable, and sometimes other 'precious materials' (gold, lapis, iron) for little details. The 'props' on the other hand are representative (somewhat) of the reason why the statue is built, and generally use whatever looks best.
Apart from general hassle in collecting material and finding and preparing a suitable location, there are a few annoying issues with converting the 'pixels' of a skin into the blocks of a statue. The first of these is the 'helm' texture. In game, this floats just on top of the head, and both are 8x8x8 in terms of 'pixels'. This means that in order to make a statue wearing a 'helm' you generally need to bump it up to 10x10x10. In some cases such as hats, this is no problem. In others such as a full helmet (or in xCoyote18's case, most of Squidward's face) this might start to overlap with the arms and upper body. The second issue is that the corner 'pixels' of the skin also overlap, meaning you have to chose which colour you want to use. These two problems are shown to the right. Finally, you have to carefully pick and choose which blocks will represent which colours. In game the grey-scale available only has 5 shades (white, light, dark, black wool and stone), 7 if you add obsidian and clay.
If you want to build a statue, take into account that they take up a substantial amount of resources. The front facing of the statue (head, arms, body, legs) will take up 352 blocks, with another 100 or so around the edge to 'carve' it into the cliff face and add depth. The props are dependent on whatever they are.

A statue was built of Scatcycle, fellow day 1 player, leader of the group 'Black Hand' (general scourge of the server for the last 3 months). Depicted is a still beating heart.
A statue was built of cliffnerd5, leading the murder tables month after month, and holding an (terribad) Ender Pearl, representing his style of minecraft combat. If there are complaints about being next to Scatcycle (couldn't be arsed finding another cliff), please direct them to my Indian call centre.
Following several months of no statues, lost time was made up with 16 of them in one huge project
